Thiamin (B1)

Thiamin is required for our bodies to burn glucose for energy. It has been found to be deficient in many overweight people. In some experiments where healthy people volunteered not to eat thiamin, the artificial deficiency caused a rise in blood glucose. In one small study, thiamin supplements reduced blood sugar and sugar spillage in urine of diabetics.

Diabetics who take insulin stimulating medications may burn high levels of sugar and use up their thiamin. Thiamin may slow the development of atherosclerosis. Thiamin is needed for nerve function, so deficiency can cause confusion, memory loss and strange behavior, a condition called Wernicke-Korsakoff syndrome. Because of its role in nerve function, thiamin may have a therapeutic use in the treatment of sensory neuropathy.

Thiamin pyrophosphate is the best form, but other forms are adequate.

Niacin (B3)

Niacin is known to be deficient in many diabetics. This is a potential problem because it has many roles in glucose metabolism. It may be a component of Glucose Tolerance Factor. (GTF binds to insulin receptors on cell membranes and makes them much more responsive to insulin by activating a molecule called insulin receptor kinase. GTF also increases insulin receptor numbers and helps insulin bond to the insulin receptor.)

Niacin improves insulin synthesis and secretion in the pancreas. Studies have shown that niacin supplements can prevent diabetes. Animal studies have suggested that niacin can protect b-cells of the pancreas from damage caused by diabetes. In studies of type 1 diabetics, niacin supplements caused a remission of the pancreatic damage lasting up to two years. More studies with diabetes patients found that niacin lowered their blood sugar. Niacin can also slow the progress of nephropathy (kidney disease). Finally, niacin supplements reduce cholesterol and triglycerides, while they increase HDL, the “good cholesterol”.

Niacin comes in two forms, nicotinic acid and niacinamide. This can become confusing because nutritionists sometimes use the word “niacin” to refer only to nicotinic acid, but sometimes to both. Both forms seem to be necessary for people who want to lose weight. Nicotinic acid seems to have a greater effect on blood sugar, while niacinamide may be better for protecting the pancreas. Only nicotinic acid improves cholesterol.

good-dietThe dosage of niacin used in successful studies varies over a large range. Some studies used as little as 30mg daily. Studies of the protective effect of niacinamide on pancreatic cells used up to 4.5gm! That is 4500mg. Studies of niacin to lower cholesterol have used 1-1.5g daily. I am suspicious of using vitamin doses hundreds of times greater than what occurs naturally in a good diet. The therapeutic dose of most vitamins is usually equivalent to what was consumed by our evolutionary ancestors. This was typically three to five times what modern Americans eat. Using unnaturally high doses, hundreds of times what would be in a good diet, seems like using vitamins as a drug. In the case of nicotinic acid, this could be harmful to your liver.

I recommend starting with about 30-100mg of mixed nicotinic acid and niacinamide daily. Use the higher doses only with medical supervision.

Some older studies found that niacin impairs glucose control. Those studies used larger doses (1-4g), another reason to stick to a more reasonable dose. Newer studies have examined this issue and seem to indicate that niacin is safe. I still stick with a modest dose, just to be safe.

Nicotinic acid, but not niacinamide can cause flushing, dilation of the blood vessels causing an unpleasant sensation. If this happens, you may not need so much niacin. Reduce your dose of niacin for a while. Wait to see if the lower dose works for you. If you have diabetes, this means waiting until your HgA1c is checked after several months. If the dose is not effective, you can gradually work up to an effective dose. Your body will gradually adjust to higher levels of niacin.
